Scalp type- oily, normal, dry, psoriasis, dandruff, etc.
Styling type- air dry, blow dry, or air dry but use hot tools..

Skin concerns- Ex: dark spots, scars, hyperpigmentation, dry spots, redness, oily, enlarged pores
Skin info- are you on any medication that would effect your skin care? Yes or No, if yes please specify which one. This helps us to know what to recommend & what results we can expect.
Do you wear makeup? If so, how often?
